Channel: Eye Channel by Eye Surgeon Dr Audrey Tai

[0]
hello everyone this is dr audrey thai
[3]
i'm a board-certified and
[4]
fellowship-trained lasik and cataract
[6]
surgeon
[6]
ophthalmologist and cornea specialist
[9]
welcome back to my channel
[10]
i channel by eye surgeon in today's
[13]
video
[13]
i would like to talk to you about my top
[15]
five choices of the over-the-counter
[17]
dry eye drops these are the artificial
[20]
tiers to treat dry eye disease
[22]
this is not a sponsored video the
[24]
products i recommend in this video
[26]
are based on my own clinical experience
[29]
treating dry eye disease patients
[31]
in my practice and the in-depth research
[34]
i did
[34]
about the science behind the ingredients
[36]
for different artificial tears
[38]
as well as my personal experience using
[41]
those eye drops
[43]
to learn more about my practice you can
[45]
follow me on facebook and linkedin
[47]
at dr audrey thai and if you have
[50]
learned anything new from this video
[52]
don't forget to like and share this
[53]
video and subscribe to my channel
[56]
so you won't miss any new videos i make
[58]
about eye health and eye surgery
[62]
as a lasik surgeon and cornea specialist
[65]
i often get patients with varying
[66]
severity of dry eye disease
[68]
referred to me from other eye doctors
[71]
for each of my patients with dry eye
[73]
disease i give them
[74]
a written customized treatment plan
[78]
all of the treatment plans include one
[80]
or more over-the-counter artificial
[82]
tiers
[83]
and i specify which type of artificial
[85]
tears that i recommend
[88]
however if you are going to a pharmacy
[90]
to choose artificial tears on your own
[92]
the choices may be overwhelming there
[95]
are typically more than 20 to 30
[97]
different types of artificial tears
[99]
available over the counter how do you
[102]
know which one is the right one for you
[105]
in this video i will go over with you
[107]
about the science behind my top five
[109]
picks of artificial tears
[111]
for treating dry eye disease and to help
[113]
you understand
[114]
the differences between them and choose
[117]
the artificial tears
[118]
that works the best for you i have
[121]
listed
[121]
the links to my top five artificial
[123]
tiers in the description down below
[126]
if you would like to learn more about
[127]
the science behind the ingredients of
[129]
different artificial tears and why i
[132]
would recommend
[132]
certain artificial tears over other ones
[136]
you can check out my video about the
[138]
science behind
[139]
artificial tears ingredients
[143]
without further ado let's talk about
[145]
artificial tears
[147]
artificial tears are intended to
[148]
supplement the natural tears that are
[150]
made by the tear glands in our eyes
[152]
the natural tears lubricate the surface
[154]
of our eyes by forming a tear film
[157]
the tear film has three layers the
[160]
outermost layer
[161]
is called the lipid layer this layer
[163]
functions as a protective barrier to
[165]
prevent tears from drying out too fast
[168]
the middle layer is called the aqueous
[170]
layer this watery layer contains the
[173]
moisture we need
[174]
to keep the surface of our eye
[175]
lubricated
[177]
the innermost layer of our tear film is
[179]
called the mucin layer
[181]
the museum layer functions as a bridge
[183]
to keep the tear film
[184]
attached to the surface of our eye it is
[187]
called the cornea
[188]
and it is the clear windshield of our
[190]
eye
[192]
an ideal artificial tear should be able
[194]
to supplement
[195]
all three layers of our tear film i will
[198]
also be posting a more in-depth video
[201]
about the mechanisms of dry eye disease
[203]
and different types of dry eyes
[205]
so make sure you subscribe to my channel
[207]
so you won't miss any of my new videos
[210]
i have divided my top five favorite
[212]
artificial tears in two groups
[214]
based on the severity of the dry eye
[216]
disease
[217]
mild to moderate and moderate to severe
[221]
and i will list the top three artificial
[223]
tiers that i recommend for mild to
[225]
moderate dry disease
[226]
as well as my top two picks to treat
[229]
moderate to severe dry eye disease
[233]
number three sustain complete made by
[236]
ourcom
[237]
it comes in a 10 ml bottle and uses
[240]
polyquad
[241]
as its preservative the reason i like it
[243]
is because it uses nano droplet
[245]
technology
[246]
which is a type of nano emulsion drug
[249]
delivery system
[250]
using negatively charged submicron size
[252]
carrier particles to provide greater
[254]
surface area
[255]
to improve the stability and absorption
[257]
of the drop
[259]
also sustain complete addresses all
[261]
three layers of artificial tears and it
[263]
contains
[264]
polyethylene glycol as its active
[266]
ingredient to form a protective layer
[269]
over the mucous membrane on the surface
[270]
of the eye
[271]
to provide hydration as well as reducing
[274]
inflammation
[276]
in addition to that it contains
[279]
hydroxypropagor
[280]
aka hbigor it is a ph dependent molecule
[283]
that can form a crosslinked network
[286]
which merges with the natural tear film
[288]
to form a protective layer to cover the
[290]
damaged epithelial cells on the surface
[292]
of the cornea
[293]
this allows damaged epithelial cells to
[296]
be able to repair themselves
[297]
in a protective space in addition to
[300]
that
[301]
sustained complete contains mineral oil
[303]
which targets the lipid layer of the
[304]
tear film
[306]
to prolong the effectiveness of the eye
[308]
drop by reducing evaporation of the eye
[310]
drop
[311]
and the aqueous layer of the tear film
[314]
this helps to retain moisture on the
[316]
surface of the eye longer
[321]
my number two pick for mild to moderate
[323]
dry disease
[324]
is refresh reliever made by allergan
[328]
it contains active ingredients of
[330]
carboxymethylcellulose
[331]
sodium or cmc as well as glycerin
[335]
both of the ingredients are
[336]
water-soluble polymer that have
[339]
characteristics similar to natural tears
[342]
and therefore they can lubricate the
[343]
surface of the eye with
[344]
minimal disruption of the natural tears
[347]
and glycerin
[348]
can decrease the osmolarity of the tear
[350]
foam on the surface of the eye
[352]
to provide additional lubrication and
[354]
promote
[355]
epithelial cell growth among the
[358]
inactive ingredients
[359]
it contains sodium hyaluronate it helps
[362]
repair corneal epithelial cells
[364]
and the lower the tear film osmolarity
[367]
the preservative used in refresh
[369]
reliever
[370]
is the purity oxidative preservative
[373]
another reason i like refresh reliever
[376]
is that it comes in both
[377]
preservative containing and preservative
[379]
free forms
[381]
one of the unique thing about refresh
[383]
reliever
[384]
is that its preservative free version
[387]
comes in a multi-dose 10
[388]
ml bottle and this packaging wastes a
[391]
lot fewer drops
[392]
and it uses much less plastic packaging
[395]
material comparing to the single-use
[396]
vial packaging
[398]
it saves money and it is convenient
[403]
my favorite over-the-counter dryer drop
[405]
for mild moderate dry disease
[407]
is refreshed digital artificial tears
[410]
made by allergan
[412]
the difference between refresh digital
[414]
versus refresh reliever
[416]
is the added emollient component in
[418]
refreshed digital artificial tiers
[421]
digital artificial tears contain castor
[423]
oil which is a natural ingredient
[425]
derived from castor beans
[427]
and it targets the lipid layer of tear
[429]
film by reducing evaporation of the tear
[431]
film
[431]
and extending the effectiveness of
[433]
artificial tears
[435]
besides cmc and glycerin an additional
[438]
active ingredient
[439]
in refresh digital is polysorbate
[442]
polysorbate can help blind the oil
[444]
component and the water soluble
[446]
component
[447]
of the artificial tears to make the eye
[449]
drop more
[450]
soluble and easily spread across the
[453]
surface of the eye
[455]
the preservative using refresh digital
[457]
is the pure white
[458]
oxidative preservative i do wish the
[461]
refresh digital preservative free
[463]
was also available in the multi-dose
[465]
bottle to reduce waste
[470]
in moderate to severe dry disease people
[472]
often need to use artificial tears very
[474]
frequently throughout the day
[476]
due to the severity of surface dryness
[479]
sometimes as often as every hour to
[481]
every two hours
[482]
it is very important for people with
[484]
moderate to severe dry eye disease
[486]
to use preservative-free artificial
[488]
tears to prevent any toxicity from such
[490]
frequent application of the
[492]
preservatives
[493]
onto the surface of the eye
[496]
one of my two favorite over-the-counter
[498]
eye drops that can be used
[499]
daily is refresh cellovist it is
[502]
preservative free
[503]
and contains the active ingredient of
[506]
one percent cmc
[508]
this is double the concentration of the
[509]
cmc contained in other types of refresh
[512]
eye drops the increased concentration of
[515]
cmc
[516]
will allow artificial tears to stay much
[518]
longer on the surface of the eye
[520]
however because of the higher
[522]
concentration of the cmc
[524]
cellular whisk is a fairly thick eye
[526]
drop and i often remind my patients who
[528]
are using it
[530]
that because the eyedrop is very thick
[532]
you may experience blue revision for a
[533]
couple of minutes
[534]
after putting it in make sure you blink
[537]
a few times
[537]
after putting the eye drop and don't put
[540]
in the drop
[541]
right before driving or operating other
[543]
machinery
[546]
my other favorite dry drop for moderate
[548]
to severe dry disease is genteel eye gel
[551]
made by alcom
[552]
genteel gel is the thickest eye drop
[554]
among my top five favorite artificial
[556]
tiers
[557]
it comes in a gel consistency and
[559]
contains hyper mellow 0.3 percent
[562]
hypermelos is short for hydroxyl propyl
[565]
macrocellulose or hpmc
[567]
it is an active ingredient that increase
[569]
the viscosity of the eyedrop
[571]
to extend the lubricating quality of the
[573]
eye drop
[574]
hpmc can cross-link when contacting the
[577]
surface of the eye
[578]
to slow down tear film evaporation
[581]
genteo gel provides long-lasting
[583]
lubrication to the surface of the eye
[586]
because of its long-lasting lubricating
[588]
quality i typically recommend my
[590]
patients with severe dry
[591]
disease to use genteel gel four times a
[594]
day including at night before going to
[596]
bed
[597]
and also because it's in a gel form it
[600]
causes less blurry vision than most of
[602]
the counter eye ointments
[604]
it is important to remember genteel gel
[606]
can still cause some blur revision
[608]
shortly after putting the eye drop
[610]
so it is important not to put in the
[613]
drop right before driving or operating
[615]
other machinery
[617]
and the preservative using gentile gel
[619]
is sodium perperate
[620]
brand name genocide it is an oxidative
[623]
type of preservative
[626]
i've listed in the description below
