Today's lesson is, we're going to talk聽 about what Medicare actually does pay聽聽

for and toward long-term care. A lot of folks that聽 come to us, and they're talking about long-term聽聽

care insurance, they're talking about Medicare聽 insurance, they're talking about financial聽聽
[16]
planning for retirement, are under the mistaken聽 assumption that Medicare is going to pay all聽聽
[21]
their long-term care, and the fact is it doesn't.聽 We're going to show you today exactly what it does聽聽
[27]
pay for. The first thing that it's going to pay聽 for a skilled nursing facility, or some folks聽聽
[34]
times folks are going to call this rehab, and聽 either you have or you know somebody that has,聽聽
[40]
where you go to the hospital typically this is聽 for older folks, you're in the hospital, and聽聽
[46]
the way they push you out of the hospital is they聽 push you across the street to the rehab facility,聽聽
[51]
and you're there a couple three weeks, now some聽 folks are there much longer, some people folks聽聽
[56]
are just there a few days. Medicare is going to聽 cover this under some rules and then of course聽聽
[62]
your Medicare Supplement insurance coverage is聽 going to pay as long as Medicare approves it, it's聽聽
[67]
going to pay as well, and so once you've met all聽 the rules, you're going to get the first 20 days聽聽

in a skilled nursing facility covered. Now聽 they're going to review you every week,聽聽
[81]
maybe every day, Medicare is and the staff there,聽 to make sure you're still Medicare certified and聽聽

Medicare is going to pay up to 100 days and聽 they're going to do those next 80 days where聽聽
[95]
if you've got some co-insurance and that聽 kind of stuff, but the real issue is is聽聽
[99]
whether they approve it or not. When you look聽 at the average of payment toward nursing homes,聽聽
[105]
it's about two percent that Medicare covers,聽 so that leaves 98 of the nursing home expenses聽聽
[112]
uncovered by Medicare. For assisted living,聽 which a lot of times folks they'll use nursing聽聽
[120]
facility or nursing home and assisted living聽 interchangeably, and there's a lot of things聽聽
[125]
about them that are similar, there's no coverage聽 from Medicare for assisted living facilities.聽聽

Adult day care, same thing, there's no Medicare聽 coverage for adult day care. Now for home health聽聽
[137]
care it gets a little bit tricky because if聽 they're doing skilled care or they're performing聽聽
[144]
what is skilled nursing care for you, and聽 you meet all the other criteria for Medicare聽聽
[151]
payment that you would have if you were in a聽 nursing home or a nurse skill nursing facility,聽聽
[156]
a bunch of rules bunch of regulations, but once聽 you qualify, then it's fairly limited what they're聽聽
[161]
going to pay for. It's this part-time care, or聽 a thing called intermittent care, where they're聽聽
[168]
coming in and doing something for you聽 and with you, like physical therapy,聽聽
[173]
that sort of thing, and you need to be home聽 bound to get Medicare to pay for it, you can't be聽聽
[180]
able to leave your house. It needs to be ordered聽 by a doctor. If you need more coverage than聽聽
[188]
part-time intermittent care, then you don't even聽 get the skilled portion of this paid for, so in聽聽
[195]
other words if you need a whole bunch of care or聽 several hours of care and you need it every day,聽聽
[201]
they're not even going to cover the skilled聽 portion of that. There's no coverage for聽聽
[205]
custodial care, which is the activities of聽 daily living, like bathing, dressing, and so on,聽聽
[212]
which is really where most of the long-term care聽 is incurred in the expense for it. Now hospice.聽聽
[219]
Medicare will cover hospice care, but in order to聽 get on Medicare hospice or to get into hospice,聽聽
[227]
a doctor needs to certify that you have聽 six months or less to live and then聽聽
[233]
Medicare is then going to get out of paying for聽 all the life prolonging care, which is what most聽聽
[239]
of medicine is, and they're simply going to聽 pay for care to make you more comfortable. So聽聽
[248]
really you're only going to get coverage in聽 three of these places, skilled, home health care,聽聽
[254]
and hospice, and then it's fairly limited聽 coverage with a bunch of rules. I'm Hans Scheil,聽聽
