Turning 65 in 2021? Medicare and Social Security Explained - YouTube
Channel: Cardinal Advisors
[0]
If you're turning 65 and coming up on Medicare,聽
today's video is really going to give you some聽聽
[6]
of the learning that I think you need to get聽
started on this whole process. So, you really聽聽
[13]
have three decisions that are critical, and you聽
need to get them right, and you need to understand聽聽
[21]
the sequence of these three decisions. So the聽
first one is going to be with Original Medicare,聽聽
[28]
Parts A and B, is whether you sign up for聽
them when you turn 65 or whether you delay,聽聽
[36]
and so you say well, what about聽
delay? Well if you're still working聽聽
[40]
and covered under group insurance you may want to聽
delay your start of Medicare Parts A and part B聽聽
[47]
until the time you leave your work, or you may聽
not, if you need the Medicare insurance or you're聽聽
[55]
not working or you're paying for insurance on聽
your own then that's a pretty simple decision,聽聽
[59]
you're going to want to sign up for聽
Medicare, Original Medicare Parts A and B,聽聽
[64]
okay, so once you make that decision now we move聽
on is how are you going to receive your Medicare?聽聽
[71]
Are you going to stay on Original Medicare or are聽
you going to purchase a Medicare Advantage plan,聽聽
[77]
which is really going on Part C, and it's getting聽
your insurance, your Medicare insurance, from a聽聽
[85]
private insurance company instead of directly聽
from the government? So that's one route to go聽聽
[90]
is the Medicare Advantage route. The other route聽
is to take keep your Original Medicare, still聽聽
[96]
get your Medicare insurance directly from the聽
government, and purchase a Supplement to Medicare,聽聽
[105]
and this is a completely different route. So聽
you're going to get your Medicare from the聽聽
[110]
government, you got fairly large deductibles聽
and copayments, or you've got liability,聽聽
[116]
and what you're going to do is you're going to聽
stay on that and then you're going to purchase a聽聽
[121]
supplemental plan, which is going to pay all those聽
liabilities for you, or most of them, so that you聽聽
[126]
have very little out of pocket. So these are two聽
very distinct routes as you're going to have this聽聽
[133]
plus this ,or you're going to have this turned聽
into this, so that's the second decision.聽聽
[141]
And then the third decision is going聽
to be once you've made those first two,聽聽
[148]
is what insurance company are you going to聽
buy your insurance from? So if you're over聽聽
[153]
here on the Medicare Supplement route, you've got,聽
we're just showing you prices around the country聽聽
[159]
for six of these companies, but聽
it's the same companies that offer聽聽
[164]
Medicare Supplement offer the Medicare Advantage聽
plans, and we're going to be into that, as well聽聽
[169]
as the standalone Part D drug plans, so those聽
are the three decisions that we need to make,聽聽
[177]
and what I'm going to do today is get into聽
each one of those decisions a little bit聽聽
[182]
and tell you what's going on so that you can聽
really make them from an educated place. So聽聽
[189]
Original Medicare is broken into two parts聽
- it's Part A, which is your hospital bill聽聽
[194]
and your hospital coverage, and Part A does聽
not cost you anything once you're on it, okay,聽聽
[200]
you've been paying taxes your whole life for聽
this and now when you're on Medicare you just聽聽
[207]
sign up for Part A and you're entitled聽
to it. Part B is a little bit different,聽聽
[211]
it is your doctor and outpatient, it's the聽
part of Medicare that pays 80% of your bill,聽聽
[217]
and then that's leaving you with 20% of the聽
bill, which is going to come out of your pocket,聽聽
[223]
and that's why people buy Medicare聽
Supplements if they stay on Original Medicare.聽聽
[228]
So you got Part A and Part B, and Part聽
B costs $148.50 a month, and most people聽聽
[237]
have this deducted from their Social Security聽
check if they're on Social Security already.聽聽
[242]
If you're not on Social Security yet, you're聽
going to have to pay this bill into Medicare,聽聽
[246]
$148.50, and if you're a high income person,聽
you've got this thing called IRMAA, which is a聽聽
[253]
an additional, it's like a surcharge, and聽
it can make this, you know, up to triple聽聽
[257]
or higher than triple, quadruple, this amount聽
when you put the two together, I mean it's just聽聽
[263]
for high income people. If you want to learn about聽
IRMAA, just give me a call get in touch with me,聽聽
[269]
I can walk you all through it. With Part B you've聽
got these lifetime penalties for making a late聽聽
[277]
sign up where you didn't do this properly,聽
so you really, if you're a working person,聽聽
[283]
now working and covered under group insurance,聽
and you've decided to delay, make that decision聽聽
[288]
cautiously because if you later get on to聽
Part B and you didn't do things properly,聽聽
[294]
you're going to have to pay a penalty and you聽
might not be able to get on it as quickly as you聽聽
[298]
want. So that's the first decision, whether we're聽
going to start this at turning 65 or we're going聽聽
[304]
to start it at some later date. Okay, so then聽
when we move into the second decision is really聽聽
[311]
whether or not you're going to take this and turn聽
it into this, which is a Medicare Advantage, okay,聽聽
[317]
or you're going to stay on Original Medicare and聽
you're going to add to it a Medicare Supplement,聽聽
[324]
okay, and all of that is outlined here. Now聽
if I'm going to help you with the decision, so聽聽
[332]
why would you want to do one or the other,聽
because this is just a clearly a one or the聽聽
[337]
other thing that you need to make before you start聽
digging into which company. So the Advantages,聽聽
[345]
or the pluses of Medicare Advantage, are a few,聽
number one is many of these come at zero premium,聽聽
[353]
so if you're able to get on Medicare A and B聽
and then outsource your Medicare to a company,聽聽
[361]
like Humana, or UnitedHealthcare/AARP, that type聽
of company, most likely you're not going to have聽聽
[368]
to pay any premium, it's a zero premium, and聽
some of them will also give you some money back聽聽
[375]
toward this Part B premium that you got to聽
make, so you get a credit back for that,聽聽
[382]
so the advantages to this is it generally costs聽
less out of your pocket, while you're healthy,聽聽
[388]
on the Medicare Advantage. You also get some of聽
these extras, like you get included a Part D drug聽聽
[394]
plan just comes with the package, you have聽
dental, you have vision, some of these offer聽聽
[400]
gym memberships, they offer food plans, there's a聽
lot of extras that come with these, okay, that's聽聽
[407]
the pluses. The disadvantages to these Medicare聽
Advantage plans is, number one, you have a network聽聽
[415]
of doctors and hospitals. So generally you need聽
to go to the doctors and the hospitals that are聽聽
[421]
on the plan and covered under the network. Now聽
most people are used to networks if they've been聽聽
[425]
on group insurance under 65, so this is not real聽
troubling, but it is a disadvantage to these plans聽聽
[432]
that you know you, you're going to have to play聽
by their rules. You also have deductibles and聽聽
[437]
co-payments on these plans, not as high as you聽
have on Original Medicare, they're much smaller,聽聽
[442]
but they still add up over time. Now the other聽
route, or the other piece of this decision,聽聽
[448]
is you're going the Medicare Supplement route and聽
that's where you have Original Medicare and then聽聽
[454]
you're adding to it a Medicare Supplement, and聽
the Medicare Supplement is going to pay this 20%聽聽
[461]
that's not covered under Part B, that's what聽
it's for, it'll even pay excess charges,聽聽
[467]
and it also, when you're turning 65, you've got an聽
open enrollment onto these Medicare Supplements,聽聽
[473]
meaning that you have to聽
answer no health questions聽聽
[477]
at all, you can, you know, we have people that are聽
on kidney dialysis, or people that have cancer,聽聽
[482]
people with diabetes, people with聽
very serious chronic illnesses,聽聽
[487]
where under normal circumstances they wouldn't聽
be able to even get a Medicare Supplement,聽聽
[492]
they're guaranteed that they can pick any company聽
they want within six months, or when they go on聽聽
[497]
Medicare, it's called open enrollment, so聽
there would be a reason to go this route聽聽
[503]
if you've got a serious chronic illness,聽
or at least to consider going this route,聽聽
[507]
is you can buy these policies without answering聽
any health questions. And then you know what we're聽聽
[513]
featured here, and what most of our customers buy,聽
is the Plan G, and the Plan G is just, A, B, C,聽聽
[521]
D,blah blah blah blah blah... has the most聽
benefits that are available to people turning 65.聽聽
[526]
Now and what we've shown here is the pricing聽
across the country, and we're started out in聽聽
[532]
New York, which you know, we're in all聽
50 states, in the District of Columbia,聽聽
[537]
we're licensed there and we can take care聽
of people anywhere in the United States.聽聽
[542]
New York is the most expensive to buy a Plan G and聽
you can look at the difference in pricing here,聽聽
[548]
is that AARP is going to charge you $268 a month聽
for a Plan G, 65 years old, in New York, and聽聽
[557]
Mutual of Omaha is going to charge you almost聽
double that for exactly the same thing - all of聽聽
[563]
these prices here, regardless of which company聽
they're with, are for exactly the same thing.聽聽
[570]
So why would you want, what are the advantages of聽
going Original Medicare and a Medicare Supplement?聽聽
[578]
So the advantage number one is you can聽
go to any doctor in the United States聽聽
[582]
and hospital that accepts Medicare. I mean聽
that's huge for a lot of folks is you're not聽聽
[589]
going to have to mess with a network, so聽
that could well be the biggest advantage.聽聽
[594]
The second advantage of these is they take聽
care of most, and the Plan G covers most聽聽
[601]
of your deductibles and co-payments, in fact when聽
I say most, there's going to be a $230 Part B聽聽
[608]
deductible in 2021, and that's going to be your聽
only liability for stuff covered under Medicare聽聽
[616]
between Original Medicare and a Supplement,聽
so there's a real clear advantage, is this is聽聽
[621]
going to pay better when you're sick and it's聽
going to let you go to any doctor in hospital聽聽
[625]
that takes Medicare, and the disadvantage is聽
you've got to pay this premium every month,聽聽
[630]
as opposed to the zero premium, so you know聽
and that may depend on which state you're in.聽聽
[636]
If you're over in North Carolina, which is where聽
our business is located and I live, this is one聽聽
[642]
of the lowest priced states in the country, and聽
you can see that there's some variance in cost,聽聽
[647]
and we're just showing these six companies,聽
there's actually about 40 different companies in聽聽
[652]
any given state that offer the Medicare Supplement聽
coverage, and they're all selling exactly the same聽聽
[658]
thing, so price is a big factor, and we're able聽
to spread sheet these things and let you decide.聽聽
[665]
Now another advantage, and then you could call聽
it a disadvantage, of going the Original Medicare聽聽
[673]
plus a Medicare Supplement is you also need to聽
purchase a Part D drug plan, so the disadvantage聽聽
[680]
is you got to pay the premium for that, $10 to $40聽
a month, some of these have actually come down in聽聽
[685]
premium over time, but what the advantage to that聽
is is we can actually plug in the drugs you're on聽聽
[695]
and we can get you with the one that's聽
going to pay the best on your drugs, so聽聽
[699]
whereas you're over here, you're not going聽
to have to pay for your Part D drug plan,聽聽
[703]
it's just going to come as part of the聽
plan, but what the disadvantage to that聽聽
[708]
is you're just going to get whatever's聽
tacked onto the plan that you want,聽聽
[712]
here you're going to have choice. Okay so then聽
the third decision that you're going to make聽聽
[719]
is, once you've decided whether you're going to go聽
here or you're going to go here and here and here,聽聽
[726]
is what company are you going to buy it聽
from, and, you know, we can offer you more聽聽
[732]
choices than these six, but these are names聽
that you're going to recognize most likely,聽聽
[738]
and many of the other companies you've not heard聽
of them before but they're perfectly fine options,聽聽
[743]
we can go over that when you call us and show聽
you different options, but I've chosen these six聽聽
[749]
because they're the largest, they're also,聽
with the exception of Mutual of Omaha,聽聽
[754]
they're the main companies that offer the Medicare聽
Advantage plans. So when you come over here,聽聽
[760]
you can look at an Aetna Medicare Advantage,聽
you can look at a United Healthcare AARP,聽聽
[766]
you can look at Humana, you can look at Wellcare,聽
and then when you're over here, you can look at聽聽
[774]
these ones plus a bunch more, and they're聽
the same companies that offer the drug plans,聽聽
[779]
and then we as well offer standalone dental聽
and vision, so, and you have to pay for that聽聽
[785]
separately if you buy a separate policy for it,聽
you're going to get some coverage for that over聽聽
[790]
here at a zero premium under the Medicare聽
Advantage plan. So we're pretty neutral聽聽
[795]
as whether which route you're going to want聽
to go, and we become less neutral once we get聽聽
[803]
to know you and find out what your preferences聽
are, what your budget is for this sort of thing,聽聽
[808]
so we provide consulting on it. Now I want to talk聽
about a couple more things before we sign off. One聽聽
[814]
is a question that we get asked many times is that聽
a lot of folks think that Social Security starts聽聽
[821]
when they turn 65, and it used to work that way,聽
and it can actually start now earlier than 65,聽聽
[832]
but we have this thing with Social聽
Security called Full Retirement Age or DRA,聽聽
[838]
and the same people that do Medicare do Social聽
Security, or the same Social Security does聽聽
[843]
Medicare, so when you're talking to them, you're聽
going to need to talk to them about when you're聽聽
[847]
going to start your Social Security, and Full聽
Retirement Age is on its way to 67. So you know,聽聽
[855]
at my age, I'm 62, my full retirement age, I聽
was born in 1958, is 66 and 8 months. People聽聽
[862]
just a little bit younger than me, it's going聽
to be 67. And for you, if you were born in 1950,聽聽
[871]
your full retirement age is 66 and 4 months, if聽
you're 1957, it's 66 in 6 months. And what full聽聽
[879]
retirement age means is that's when you're going聽
to get your full benefit of Social Security.聽聽
[885]
Now you still may want not want to start on that聽
date, you may want to delay all the way to 70聽聽
[889]
because you'll get a larger check, but you're聽
also going to miss out on those checks.聽聽
[894]
Some of you may already be on Social Security聽
because you started early, then this whole聽聽
[899]
explanation is kind of a mute point. There's also聽
factors with Social Security about your spouse, is聽聽
[905]
that many of you will collect on your spouse who's聽
the higher earner. If that's the situation, if you聽聽
[912]
have a deceased spouse and you didn't remarry聽
before 60, you can collect off of their benefit,聽聽
[918]
divorce spouse the same thing, different benefit,聽
so there's a lot of consulting that can be done聽聽
[924]
on that, and what I'll add on this video is age 64聽
is a great time to be looking at your IRA and 401k聽聽
[934]
and seeing now I'm going to need to turn that聽
into an income in retirement, how do I do that?聽聽
[939]
We can certainly help you with that. Life聽
insurance it's a point where it's Life Insurance聽聽
[945]
is still affordable and if you've been relying聽
on group insurance, or you have term insurance聽聽
[949]
that's expiring, we can get you a smallish policy聽
or smaller policy or even a large policy that's聽聽
[956]
going to be enforce for the rest of your life.聽
And then we need to have the discussion about聽聽
[960]
long-term care insurance and who's going to take聽
care of you when you can't take care of yourself聽聽
[965]
if that situation happens to you. I'm Hans聽
Scheil and I thank you very much for listening.
