and check it out in that video we talked all about聽 creating a workflow for dealing with things like聽聽

the no surprises act and i've now done my research聽 i've talked to people about the no surprises act聽聽

and i've learned that i need to implement聽 something called the good faith estimate聽聽

this is a form that i have to give to clients聽 specifically out of network clients that describes聽聽

a whole bunch of different things and so i decided聽 this would be a good opportunity to teach you how聽聽

to create custom forms inside of simple practice聽 so the reason why i've opted to create my own form聽聽

rather than use a template is because i personally聽 like to know every single detail on the forms that聽聽

i give to clients and so by creating it myself it聽 helps me to learn it a lot better so anyway today聽聽

i'm going to show you the process of actually聽 creating the form you'll see a bit about what聽聽

information i used to put into the form and聽 in the description below i'll add links to all聽聽

the information that i used in order to create聽 this form but before we hop into the computer

all right so here we are in simple practice聽 and now just keep in mind you can do this聽聽

probably in any ehr software just so you聽 know you can see here i have a split screen聽聽

and on the right hand side you are going to see聽 my actual template this is what i made inside of聽聽

simple practice and then i downloaded it and聽 i have put it next to my other screen just聽聽

as reference all right so that's actually what聽 we're going to create right so on the left hand聽聽

side here we are in our ehr and so what we're聽 going to do we're going to go down to settings聽聽

we're going to go to notes and forms and then聽 we're going to go to create new template okay聽聽

and so what we're going to do we're going to name聽 the template we'll put gfe good faith estimate聽聽

and now here you want to put this under at聽 least for a simple practice you want to put聽聽

this underneath assessment so specific to simple聽 practice when we make something an assessment you聽聽

can pull it up regardless of whether or not聽 the client has an appointment and since we聽聽

need to give this document to a client before聽 any appointment is scheduled it's going to be聽聽

important to be able to do that so we're going聽 to start by clicking add new and you can see聽聽

on my reference sheet a couple of things that we聽 need to know are just patient demographics right聽聽

so i like to just stick with long answer which of聽 course we don't need a long answer for someone's聽聽

name but just to give someone extra room and聽 flexibility i like to choose long answer multiple聽聽

line text and the question would simply be the聽 prompt you can put patient name and then you're聽聽

going to push save okay and then we're going to聽 do this again so we're going to go to add new聽聽

we see here that we also need a patient's聽 date of birth and here we can actually choose聽聽

date which is right here okay and then question聽 again which would be the prompt you can put聽聽

something like patient date of birth and then聽 we're gonna click save so what you're gonna do聽聽

is continue to add different elements to the form聽 based off of what you need to have in that form聽聽

all right so we have patient name patient date of聽 birth and then there's also going to be something聽聽

called description of psychotherapy services聽 there's going to be something called services聽聽

that are reasonably expected to be furnished聽 you get the point i'm not going to go through聽聽

the entire good faith estimate with you links to聽 all the information about that that i used will聽聽

be down below in the description but essentially聽 you'll continue to do add new and you can choose聽聽

whatever option works best for you for the most聽 cases because i like the flexibility i'm usually聽聽

choosing long answer multiple line text however聽 if something is more convenient for you or works聽聽

better feel free to choose that and try it out聽 now there is a section also that requires you聽聽

to enter your information and since this will not聽 change you can actually do this a bit differently聽聽

so the first thing you need to have in here is聽 your name right and this will most likely not聽聽

change so what you can do to make this easier for聽 you is you can choose single choice radio buttons聽聽

and then for the question like before you can聽 just type in provider name that will be the prompt聽聽

you're then going to click answer and you're going聽 to put in your name and then you're going to click聽聽

save let's go ahead and preview this real quick聽 you're going to see here under provider name聽聽

there it is there's nothing you need to fill out聽 you don't need to type that over and over you can聽聽

simply just click that radio button and boom you聽 have your your name there and there's a few other聽聽

sections you can do just like this such as your聽 tax identification number and npi no need to type聽聽

these over and over again you can simply click the聽 radio button and have it pop up automatically and聽聽

then finally there's a section on the good faith聽 estimate for disclaimers and these disclaimers聽聽

are not something you're going to fill out nor聽 are they something a client is going to fill聽聽

out they're simply paragraphs or text that you're聽 going to have on your form so what you're going to聽聽

do is go to add new and you're going to go ahead聽 and click paragraph text no client input and聽聽

you're simply going to type out your disclaimer聽 then you're going to click save let's preview that聽聽

real quick and here we go this obviously is not聽 the good faith estimate but it is essentially the聽聽

building blocks so that's pretty much it hopefully聽 you know a little bit more about the good faith聽聽

estimate now and the importance of creating custom聽 forms within your ehr software i hope you learned聽聽
