one you absolutely need to understand i'm going聽 to tell you right now why i would never choose a聽聽

i am not a fan of medicare advantage i'm going聽 to tell you exactly why i personally would never聽聽

choose a medicare advantage plan so first we need聽 to understand what is a medicare advantage plan聽聽

and it really should be called medicare managed聽 care because that's exactly what it is so if you聽聽
[52]
choose a medicare advantage plan and these are聽 all the tv commercials that you see especially聽聽

at the end of the year during medicare's open聽 enrollment plan primarily all medicare advantage聽聽
[63]
plans so what a medicare advantage plan is is if聽 you choose to go on to a medicare advantage plan聽聽

it takes you off of original medicare and put you聽 into a private insurance carrier's managed care聽聽

plan now how does that work and why does the cost聽 not reflect that to you medicare actually pays a聽聽

managed care plan to take responsibility聽 of all your medical claims so they pay聽聽
[92]
it's generally upwards of about a thousand dollars聽 a month per member to the medicare advantage聽聽
[97]
company to take your claim responsibility聽 away from medicare so good for medicare聽聽
[103]
and good for the medicare advantage company聽 because they want as many as possible that's why聽聽
[109]
you see so many commercials and why they sell聽 so hard because for each member that they get聽聽
[114]
they get a significant monthly premium that goes聽 along with it so when you go into a medicare聽聽
[120]
advantage plan again it ought to be called managed聽 care used to be called in the old days an hmo聽聽
[126]
health maintenance organization which you聽 don't hear that term that much anymore聽聽
[130]
but that is essentially what most of these聽 plans are and it takes the decision making away聽聽

from medicare to the medicare advantage company聽 so my experience with managed care plans in my聽聽
[144]
younger days i actually worked for four different聽 managed care companies i was an account executive聽聽
[151]
and i did manage care sales primarily to large聽 employers so if you work for a large employer your聽聽
[158]
group health plan was typically an hmo and you may聽 have had some other options but that was typically聽聽
[166]
what your primary choice was at a large employer聽 and i was the one that would go in every year and聽聽
[172]
explain the benefits and tell them how much their聽 costs have increased and their benefits decreased聽聽

it really was a miserable job it was not enjoyable聽 i happen to be good at it which worked out okay聽聽
[185]
but it just was not something that was in a聽 place of feel good so i have made the transition聽聽

since then about 10 years ago into medicare聽 medicare is absolutely a feel-good position聽聽
[199]
and it's very enjoyable when you do it correctly聽 because it does exactly what it's supposed to do聽聽
[205]
medicare is probably possibly the only government聽 program that does exactly what it's supposed to聽聽
[211]
do and does it very well and that's why it's聽 confusing to me why anyone would want to choose聽聽
[217]
to leave medicare and go to a medicare advantage聽 plan unless it's kind of a sales situation where聽聽
[224]
somebody's selling you into that scenario so i'm聽 going to tell you exactly why i would never choose聽聽

that and there's a couple of main reasons let's聽 first talk about the cost differences between a聽聽
[235]
medicare advantage plan and staying on original聽 medicare so if you stay on original medicare a聽聽
[240]
and b you'd likely have a medicare supplement plan聽 that covers the pieces that a and b do not a has a聽聽
[247]
deductible that's kind of big b has a deductible聽 that's pretty small and then there's some 20聽聽
[253]
co-insurance also that you wouldn't want to be聽 responsible for so you get a medicare supplement聽聽
[258]
plan that covers most of all of that and a聽 medicare supplement plan generally for the聽聽
[264]
folks that we deal with runs pretty close to about聽 a hundred bucks a month so it's not by any means聽聽
[270]
prohibitive in a cost standpoint now something to聽 understand whether you're on original medicare or聽聽
[276]
on a medicare advantage plan you're always going聽 to have your medicare part b premium so your part聽聽
[283]
b premium which is also a couple hundred bucks聽 a month is going to be paid regardless of which聽聽
[289]
avenue you choose so that really negates the聽 difference between the choice because that's聽聽
[295]
always there the difference after that is the聽 medicare supplement plan and then on original聽聽
[301]
medicare you'd also probably get a medicare聽 drug plan so your medicare drug plan can run聽聽
[307]
as low as 10 a month up to about 20 25 a month聽 depends on how complicated of prescription drug聽聽
[314]
regimen that you have now a medicare advantage聽 plan can have zero additional premium or it can聽聽
[322]
have 50 a month or it could be higher than that聽 it also generally will include medicare drug plan聽聽
[328]
as part of the medicare advantage part of it聽 now something to understand if you are looking聽聽
[333]
at medicare advantage plans the lower that cost聽 is generally the more restrictive it will be or聽聽
[341]
the smaller network of doctors and hospitals so聽 kind of get what you pay for without a doubt so聽聽
[349]
when you go to use either original medicare聽 with a supplement or medicare advantage plan聽聽
[354]
significantly less expensive with original聽 medicare and a supplement when you have聽聽
[359]
a supplement let's say you have plan g with聽 your a and b you're out of pocket for the year聽聽
[365]
is a couple hundred bucks and that's it for the聽 entire year once you're out of pocket a couple聽聽
[370]
hundred bucks which is generally just the medicare聽 part b deductible it pays a hundred percent the聽聽
[376]
rest of the year without any out-of-pocket expense聽 on medicare advantage it's kind of a pay-as-you-go聽聽
[384]
and your out-of-pocket maximum for the year if聽 you had a really significant year medically can聽聽
[390]
be as high as 7 500 for the year so absolutely聽 something to keep in mind can be a little bit聽聽
[397]
less expensive on the front medicare advantage聽 but can be significantly more expensive as you聽聽
[403]
use it so if you never use it it's terrific and聽 that's generally where you hear the good things聽聽
[408]
about medicare advantage is people that聽 haven't had a lot of experience using them聽聽
[413]
if you go on to google and you pick a medicare聽 advantage company and you look for reviews聽聽
[418]
on that company i mean you'll find books聽 and books of reviews on medicare advantage聽聽
[423]
medicare supplement there's really no interaction聽 with an insurance company you go to the doctor聽聽
[430]
you give them your medicare card you give them聽 your supplement card the doctor bills medicare聽聽
[435]
medicare pays their part and sends it then to聽 the medicare supplement company the medicare聽聽
[441]
supplement company has no decision to make聽 on whether they cover or whether they don't聽聽
[446]
cover if medicare covers it they have to cover聽 it so there's literally no interaction between聽聽
[452]
you and the insurance company or any聽 pre-authorizations or any referrals聽聽
[457]
or anything of that nature with original聽 medicare that's the primary reason聽聽
[461]
that i would absolutely choose original medicare聽 would never choose medicare advantage is because i聽聽
[467]
want control over what i do and medicare original聽 gives you that control it's absolutely terrific聽聽
[474]
it's basically autonomous if it's medically聽 necessary and your doctor says you need it聽聽
[480]
medicare covers it and then your supplement has聽 to cover it because they have to cover whatever聽聽
[486]
medicare covers so there's literally no decision聽 made on a medicare advantage plan different story聽聽

there's pre-authorizations where if you're going聽 to have something significant generally they have聽聽
[498]
to get it pre-approved before you can do it and聽 that can cause wait times of weeks even months聽聽
[505]
to have whatever procedure you need to have done聽 done and especially in these days where things聽聽
[511]
get a little more complicated with wait times also聽 authorizations and referrals so referrals from a聽聽
[520]
primary to a specialist that's probably the number聽 one complaint with a medicare advantage company is聽聽
[525]
if you need to go to a specialist first you have聽 to go to a primary care ask them if it's okay they聽聽
[531]
give a referral then you make an appointment with聽 a specialist it can add weeks to the time that you聽聽
[537]
get to actually see that specialist because you聽 had to make extra trips to a primary care first聽聽
[542]
original medicare you don't need any of that now聽 sometimes a specialist may require that you see a聽聽
[549]
family doctor before you come to see聽 them that's very possible if it's a聽聽
[553]
neurologist or something like that they want聽 you to make sure you know why you're coming聽聽

but you don't have to do it it's not a requirement聽 so it's much easier to get through things and to聽聽
[565]
choose what you want to have done and leads me to聽 the next point of networks and networks probably聽聽

the number two complaint on medicare advantage聽 when you join a medicare advantage plan you聽聽
[576]
primarily have to use a network of doctors and a聽 network of hospitals that are based generally in聽聽
[583]
your local area so and again can be cost related聽 so if it's a zero dollar plan that you're in you聽聽
[590]
probably have a much smaller choice of doctors a聽 much smaller choice of hospitals if you're paying聽聽
[596]
fifty dollars a month or eighty dollars a month in聽 premium you might have a bigger network of doctors聽聽
[601]
and a bigger network of hospitals but聽 in your local area again it does not聽聽
[608]
go outside of your area for the most part on聽 a medicare advantage plan if you are traveling聽聽
[613]
and you run into a medicare or medical scenario聽 and it's an emergency it's going to be covered聽聽

and then generally if it's going to require聽 continued care they're going to require you to聽聽

come back within the service area another reason聽 that i would never join a medicare advantage聽聽
[632]
plan with original medicare you can go to any聽 doctor any hospital anywhere in the country any聽聽
[639]
50 states that accepts medicare without asking聽 if it's okay to go so if you want to choose to go聽聽
[645]
to a specialist for your very unique situation聽 in new york or in california or in nevada聽聽
[653]
you simply make the appointment and do it you聽 don't have to get authorization on a medicare聽聽
[659]
advantage plan you cannot do that now there聽 are medicare advantage plans that will have聽聽
[664]
out of network benefits we have out of聽 network benefits that allow you to go聽聽
[669]
other places the issue with that is cost and聽 generally with any out of network benefit you聽聽
[675]
will find that it is cost prohibitive meaning it's聽 super expensive to go out of network number one聽聽
[682]
the out of network doctor can charge聽 whatever they want to charge they have no聽聽
[687]
pre-negotiated rates with an insurance company聽 and then the medicare advantage plan would pay聽聽
[694]
based on usual and customary which could be far聽 less than what that doctor actually is charging聽聽
[701]
and therefore you will pay that balance bill聽 after the usual and customary part of it is taken聽聽
[708]
care of and it can get super complicated with聽 medicare original none of that it's all already聽聽
[715]
pre-negotiated doctors that accept medicare agree聽 they have to accept medicare approved charges聽聽
[720]
and that's how they get paid and then again your聽 supplement again doesn't play any part in the聽聽
[725]
decision factor you go to the doctor you give聽 them your medicare card they send the claims to聽聽
[729]
medicare medicare then sends the balance to聽 the supplement that they're supposed to pay聽聽
[735]
and that's it there's no questions asked i have聽 thousands of clients on medicare supplements聽聽

i get zero customer service calls literally聽 zero if if i get a call it's about a lost聽聽
[746]
card or something like that but never i mean i聽 can't recall last time that somebody said hey聽聽
[752]
this didn't get paid and this is a problem it聽 just doesn't happen and to be perfectly clear聽聽

and transparent i sell medicare supplements聽 i'm an insurance broker that's what i do聽聽
[763]
i could absolutely easily sell medicare advantage聽 plans medicare advantage plans actually pay a much聽聽
[769]
higher commission than medicare supplement plans聽 but i i completely uncomfortable with selling聽聽
[775]
something to somebody that's super important聽 in something that i just don't believe in so聽聽

that's generally why i focus solely on聽 medicare supplements because it's terrific聽聽

it's easy it's great for me too because i don't聽 get a bunch of angry customers and it does exactly聽聽

what it's supposed to do i get calls all the time聽 from friends that i know that my sister's on this聽聽
[799]
medicare advantage plan and now there's an issue聽 and we need to get her off how do we get her off聽聽
[804]
you can't get her off you once once you're in a聽 medical scenario and you're already in medicare聽聽
[810]
in most states you'd have to go through medical聽 underwriting to get a medicare supplement plan so聽聽
[816]
it's not the way it is now if you're on a medicare聽 supplement for some crazy reason you wanted to go聽聽
[820]
to a medicare advantage plan you can do that聽 anytime at the end of the year during medicare聽聽
[825]
open enrollment but you can't go from a medicare聽 advantage plan back to a medicare supplement聽聽
[831]
in most states there are a handful of states that聽 allow you to do it but in most states you have to聽聽
[837]
go through medical underwriting so you don't want聽 to wait until there is a medical scenario where聽聽
[842]
you need to go to see a certain cancer doctor聽 in north carolina or a certain cardiologist in聽聽
[848]
arizona you just don't want to be in that position聽 and then have your hands tied because you can't do聽聽
[855]
it with original medicare you can do it you can聽 go anywhere you want you can do anything you want聽聽
[860]
and there's no questions asked on why do you聽 want to go there and that again is a main reason聽聽

that i would choose always original medicare聽 and i would never choose medicare advantage so聽聽
