about a question how long does medicare

pay for home services

jonathan you're the expert in the area
[14]
go for it yeah so
[16]
when a physician writes a script for
[18]
home care services that script is deemed
[21]
uh it's eligible for 60 days so a lot of
[24]
times people think that they can only
[25]
receive services for 60 days
[28]
but the really nice thing about what
[30]
medicare uh believes in is that if a
[33]
patient
[34]
it's deemed necessary that they may need
[37]
more skilled services beyond 60 days
[39]
what they allow is for a
[41]
re-certification for
[43]
another 60 days an easy example of that
[46]
is if there's someone with a wound a
[47]
woman that needs tending to
[49]
that may not heal within 60 days and so
[51]
our nurses will continue as long as it's
[54]
justified if it's documented
[56]
appropriately we technically can be on
[58]
services for as long as there's a
[60]
medical justification so 60 days is the
[63]
time frame but we can always extend by
[65]
60 days still have to meet medical
[67]
criteria for it to be covered so this
[69]
means a doctor has to write a
[70]
prescription that you actually do need
[72]
home care and must certify your care
[74]
that means writing a prescription
[75]
sending it to the home care company
[77]
and you must be homebound so a lot of
[81]
the times is technically everybody could
[83]
make it to the doctor's office if an
[84]
ambulance picked you up and a crew of
[86]
six people picked you up but generally
[88]
if you can make a reasonable case that
[90]
it's extremely hard you don't have a car
[92]
you're crippled you're in a wheelchair
[95]
these things combined with a doctor's
[97]
prescription can qualify you for these
[101]
things at your house so skilled nursing
[103]
hair that means wound care changing iv
[105]
lines you know special care options
[107]
that's a couple times per week physical
[109]
therapy occupational therapy which means
[112]
like how to move around the house how to
[114]
use your upper body social services
[117]
that's like evaluating you or helping
[119]
you with your life circumstances at home
[122]
health aid there's also durable medical
[124]
equipment so like hospital beds
[127]
wheelchairs walkers certain types of
[130]
braces i know logistically this isn't
[132]
the easiest thing to do at the house but
[135]
these are things that are covered and
[137]
we're talking like once twice or three
[139]
times a week uh if you're able to get
[142]
that coverage uh getting a nurse out
[144]
there this is not 24 hour care
[146]
unfortunately so some people after a
[148]
stroke with dementia
[151]
basically do need longer term care this
[153]
is considered custodial care that is
[156]
generally not covered by medicare and
[159]
that's kind of the difference is
[161]
knowing where that line is and i'll talk
[163]
about that later in the video but
[165]
skilled nursing care is the services in
[167]
the list i mentioned
[169]
maybe a few times per week for what's
[171]
called intermittent or short-term care
[174]
and the big thing for my end you know i
[176]
do a lot of wound care for the home
[179]
uh
[180]
companies like jonathan's park home care

realistically a wound will not heal in
[186]
one month so a lot of the people on at
[188]
home that need wound care on average
[190]
even if everything is going perfectly a
[192]
wound closes maybe 50
[195]
in a month you know so it can sometimes
[198]
take three to six months or more to heal
[200]
a wound same thing with a broken leg a
[203]
broken foot a motor vehicle accident
[205]
we're talking months would you be able
[206]
to help people out like that if the
[208]
doctor are recertified absolutely
[211]
absolutely if there's a if there's a
[212]
justified need which it helps when the
[214]
physician also sees there's a medical
[216]
need for it we will go in there and
[218]
we'll carry out the prescription that
[220]
the physician wants us to do and we will
[222]
service that patient whether they need
[223]
therapy or they need nursing services we
[225]
could carry it out yeah and i think just
[227]
in the meantime a doctor has to write a
[230]
certificate or
[231]
the script but see you in person

so sometimes you know jonathan's company

can get people out to see you guys in

the metro detroit area or you can make

it in a clinic once every once in a
[242]
while to get checked on your progress

and make sure it's doing good right

great an alternative to intermittent
[248]
care or covered medical care is what's
[251]
called custodial care or private duty
[254]
care so this includes things like
[256]
helping around the house meals homemaker
[258]
services bathing dressing using the
[260]
bathroom personal grooming watching your
[264]
loved one respite care so these are
[266]
companies like assisting hands which we
[269]
work with closely as well if you have
[272]
dementia or stroke or you need extra
[274]
help that your family can provide
[277]
more of a private duty
[278]
coverage service can be beneficial if
[281]
skilled care cannot cover your needs and
[284]
this includes stuff like getting a ride
[286]
to your doctor's appointment so if

dietitians that can come out nurses that

can come help you uh there is a

difference though if you need somebody

there permanently like eight hours a day

you know 12 hours a day or maybe four
[321]
hours a day to help you get to your
[322]
appointment and give you a ride that's
[324]
where private pay home care services can
[327]
