Dearborn National administers the Life and Accidental Death & Dismemberment benefits

for UT System employees and their dependents.

As an active employee enrolled in the UT SELECT medical plan, you will automatically receive

$40,000 of Basic Group Term Life Insurance and $40,000 of Basic AD&D coverage at no additional

cost.

Voluntary Group Term Life Insurance is available to you in increments of 1 to 10 times your

annual salary, up to a maximum of $2,000,000.

New employees can select up to three times annual salary without completing an Evidence
[37]
of Insurability (or EOI) application.

If you increase your salary multiplier during annual enrollment or mid-year because of a
[45]
qualified change in status, EOI approval may be required.

The Premium rate is based on your age as of your effective date of enrollment and each

subsequent September 1.

Active Employees can select Dependent Voluntary Group Term Life for their spouse and/or children.

You have an option of $10,000 for your spouse and each eligible child without EOI approval.

Additional Voluntary Spouse Group Term Life of either $15,000 or $40,000 can be selected,

but it will require EOI approval.

The premium rate is based on your spouse's age as of the effective date of enrollment

features of your Group Term Life Insurance plan including options for keeping coverage

should you leave employment with UT, and a waiver of premiums should you become totally

disabled.

In addition an accelerated death benefit is also available, if you are diagnosed as terminally

ill.

Voluntary AD&D is available to active employees in amounts equal to multiples of $10,000 or

up to 10 times annual salary.

The maximum coverage you can have is $2,000,000.

No EOI is required for AD&D.

The spouse of an active employee can have voluntary AD&D up to 50% of the employee's

voluntary AD&D coverage amount, in multiples of $10,000, up to a maximum of $1,000,000.

As the employee, you must be covered for at least $20,000 of AD&D coverage to be eligible

to elect voluntary AD&D coverage for your spouse or children.

Your eligible children can each be covered for $10,000.

Total paralysis of upper and lower limbs.

This Benefit is based on serverity of loss.
