[6]
first let's look at some normal children
[8]
in nursery school
[10]
recalling normal behavior will help in
[12]
recognizing the abnormalities of the
[14]
psychotic child
[16]
most children of this age enjoy being
[19]
around each other
[21]
they like to watch and rub elbows
[26]
normal children are very aware of the
[28]
teacher are interested
[30]
in her and attentive to her activities
[40]
imitation is common in healthy preschool
[43]
relationships
[49]
three-year-olds cooperate in play and
[51]
show a lively interest in each other
[63]
play is meaningful and full of emotion
[67]
although we cannot hear them be assured
[70]
that they are
[70]
a talkative noisy giggling squealing
[73]
group
[85]
no matter how simple the play of the
[87]
normal three or four year old may appear
[90]
it has a definite purpose
[95]
both normal and abnormal children attend
[97]
the nursery school we see here
[112]
john entered nursery school at three
[114]
years of age
[116]
he had no language note the peculiar
[119]
flapping of hands
[120]
as he dances on tiptoe
[125]
initially much of his time was spent in
[127]
aimless wandering
[130]
seemingly watching nothing in particular
[141]
he could easily be led to the swing
[144]
because this was the only activity
[146]
he seemed to enjoy
[155]
he was often frightened by the sounds of
[157]
normal children at play
[159]
and by the camera
[164]
john effectively avoided the other
[166]
children and managed to be by himself
[171]
any interest in play materials was brief
[175]
the teachers were aware of his illness
[177]
and spent a good deal of time with him
[179]
giving support
[181]
here he exhibits one of his typical
[182]
mannerisms
[184]
tapping his lip with his finger and
[186]
waving his hands
[196]
he never initiated change so had to be
[199]
led
[200]
indoors or outdoors indoors
[203]
he often paced about in a repetitive
[206]
pattern
[210]
or just stared into space
[221]
when an occasional child approaches him
[223]
he pays no attention
[225]
he does not speak he just wards off the
[228]
intruder
[229]
he is unable to relate to children in
[232]
any normal fashion
[234]
his habit of climbing onto the toy
[235]
cabinet and remaining there for long
[238]
periods
[239]
may have served to isolate him from the
[241]
others
[243]
a lack of normal relationships with
[245]
others is a cardinal sign of infantile
[252]
psychosis
[257]
he takes some raffia from the nearby
[259]
flower basket
[260]
and shakes it and chews it
[267]
he could engage in such monotonous
[268]
activities for long periods
[287]
or he isolates himself on the table then
[290]
finger taps
[292]
abnormal motility is best seen in his
[294]
pacing
[298]
he also liked to twirl in circles
[304]
a disturbance in motility is also a
[306]
predominant sign of infantile psychosis
[309]
here the abnormality is noticed in the
[311]
hand movements
[314]
although he would occasionally approach
[315]
a toy
[317]
he always stopped short of using it for
[320]
its intended purpose
[326]
still another significant sign of
[328]
infantile psychosis
[329]
is disturbed relationships with objects
[332]
or play materials and toys
[335]
john simply mouse the pegs instead of
[338]
placing them in the pegboard
[344]
then he strokes the toy with a
[346]
repetitive finger movement
[381]
these home movies of john beginning at
[383]
12 months of age
[385]
indicate a deceptively normal appearance
[388]
he was capable of quite a show of
[390]
emotion especially with his father
[393]
most of the time his face had little
[395]
expression
[398]
with his mother he was always distant
[404]
the parents history and these pictures
[406]
at 12 months show less responsiveness
[409]
than expected in the normal
[423]
his mother preferred not to hold him for
[425]
feedings
[426]
she fed his bottle in this manner
[432]
absence of anticipatory posture to being
[434]
picked up is a frequent early sign
[443]
a peculiar mannerism was stroking the
[448]
floor
[451]
he approaches a toy but taps at it
[454]
instead of reaching for it and handling
[456]
it as one might expect
[465]
he apparently liked to stroke the floor
[467]
for the sound it produced
[475]
at 18 months he spoke no words but his
[477]
hearing was normal
[478]
as indicated by his ability to locate
[480]
airplanes
[483]
here at 18 months in a public park
[486]
he is fearful of these children
[492]
in his distress he fails to recognize
[494]
his mother
[495]
but crawls to a strange woman
[500]
at age two he is in the midst of toilet
[502]
training
[503]
he can say hi daddy at this age but
[506]
later loses these words
[509]
here we will see an abnormal motility
[511]
pattern already evident
[513]
as he runs note his rapid hand movements
[521]
now almost three john still wishes to
[524]
avoid his mother
[526]
he prefers to remain at a distance
[531]
it was not until two and a half that
[533]
john's parents became concerned and
[535]
suspected him abnormal
[536]
when language had failed to develop
[555]
although the outlook for recovery from
[557]
infantile psychosis is very poor
[560]
we can observe some changes and
[562]
improvement in john
[563]
during his year at nursery school music
[566]
became his first sustained interest
[569]
his attention is noticeably drawn when
[571]
records are played
[573]
the music seemed to soothe him
[580]
after a year in school he would even get
[582]
on the rocking horse
[588]
the face we see now makes it hard to
[590]
believe he was so animated at 12 months
[599]
now it's difficult to imagine what is
[601]
going on in his mind
[604]
it was not until john had been in
[606]
nursery school for almost a year
[608]
that he could be interested in finger
[610]
painting
[616]
now he shows an awareness of this play
[618]
material and the teacher's attention
[620]
which is a bit late
[640]
his play here represents quite an
[643]
improvement in his relationship to
[644]
materials
[645]
and toys
[661]
he notices the paint fall to the floor
[673]
he realizes he has a finished product
[687]
the camera catches a rare glimpse of
[688]
curiosity
[695]
john was seen to make occasional bids
[697]
for attention from the teacher
[705]
one teacher in particular played with
[707]
him on a level that he really enjoyed
[714]
twirling was his favorite activity
[718]
this is the way he showed his best
[719]
relationship with the teacher
[725]
he made screaming sounds when he was
[728]
pleased
[731]
he made some progress in relationships
[733]
with other people too
[735]
he came to accept men especially even
[738]
though
[738]
total strangers
[741]
he would lean against them or crawl into
[743]
their laps
[746]
however he avoids face-to-face meeting
[750]
he manages to maneuver himself back into
[752]
a position facing away from the person
[758]
seemingly delighted with his new
[760]
acquaintance john dances merrily about
[762]
the room
[776]
then as if motivated by the attention he
[779]
is receiving
[780]
he evidences appropriate play as he
[783]
steers a small car about the floor
[788]
this is the first time he has engaged in
[790]
this type of play
[797]
now four years of age john leaves school
[800]
on one of his last days
[802]
he still has no language and is not yet
[805]
toilet trained
[806]
however he is much more aware of his
[808]
environment than he was a year ago
[812]
although not normal he can now make some
[814]
relationships with people
[816]
and play appropriately with some
[818]
materials
[819]
unfortunately the ultimate prognosis
[822]
remains poor
[823]
in infantile psychosis even under the
[825]
best and most
[826]
intensive treatment
