started one of the things we get asked a

lot about is I've been working since I
[46]
turned 65 I had employer coverage or I
[48]
had employer coverage to my spouses
[50]
employment and I've just now lost that
[53]
coverage
[54]
well after age 65 how do I start my Part
[57]
B coverage well it's been a big usually
[61]
it's not that big of a deal you just
[63]
fill out the form CMS 40 B and L 564
[66]
take them to Social Security office and
[68]
they process the paperwork but nowadays
[70]
we've got this little bit of an issue
[72]
with Medicare offices or excuse me
[75]
Social Security offices having limited
[78]
hours or maybe even being closed because
[79]
of coronavirus so let's take a look a
[82]
little bit at the process and I'll walk
[84]
you through step-by-step if you're
[86]
already enrolled in Medicare Part A so
[88]
you've got your hospital coverage with
[90]
no premium you paid for it while we were
[92]
working you delayed your Part B because
[95]
you had employer coverage you didn't
[96]
want to pay that premium then you need
[98]
to complete the form CMS 40 B is simply
[102]
a one-page application for Medicare Part
[105]
B coverage and we have those forms on
[107]
our website you'll find links in the
[109]
description box below

what you're filling that out state I
[113]
want my Part B coverage to begin the
[116]
month and the year don't worry about a
[118]
day it's always the first day of the
[120]
month with Medicare in the remarks
[122]
section of that CMS 40 B or there is a
[125]
new online application if you are
[128]
applying for Medicare Part B due to a
[131]
loss of employment or group health
[133]
coverage
[133]
yours or your spouse's employment then
[136]
you need to have the employer complete
[138]
the CMS l 564 a request for employment
[142]
information here's the change if your
[146]
employer is unable to complete section B
[148]
of CMS L 564 or you just don't want to
[152]
wait to get it back you can complete
[154]
that portion as best as you can on
[156]
behalf of your employer without your
[159]
employer's signature what we do is
[162]
you'll need to submit one of the
[165]
following six forms of secondary
[167]
evidence of group health insurance
[169]
coverage this could be income tax
[172]
returns that showed health insurance
[173]
premiums paid it could be a w-2
[176]
reflecting pre-tax medical contributions
[179]
it could be a pay stub that shows they
[182]
deducted money from my check for health
[184]
insurance it could be a health insurance
[186]
card as long as it shows a policy
[189]
effective date not all do and an
[192]
explanation of benefits paid by the
[195]
group health plan so this is that
[197]
statement it says the insurance got the
[199]
bill from dr. Smith and this is what
[201]
they pay this is your your obligation or
[203]
recent statements or receipts that
[205]
reflect payment of those group health
[207]
insurance premiums pretty easy now
[211]
you'll have three ways to submit your
[214]
CMS 4tb which is just your one-page
[217]
application for Medicare Part B and that
[220]
CMS L 564 the employer form with any
[224]
proof needed number one you can mail
[227]
your forms directly to your local Social
[229]
Security office now we recommend making
[232]
copies of what you mailed just in case
[235]
number two you can fax your forms and
[238]
you see the fax number they're listed or
[240]
number three if you're online you want
[243]
to get it done quicker apply online for
[246]
Medicare Part B during the special
[248]
enrollment period there's a link and
[250]
we'll take youth walk you through that

here in just a minute so regardless of
[255]
which of the three methods you use after
[258]
you apply CMS the federal agency that
[261]
manages the Medicare program will send
[263]
you a welcome to Medicare packet with
[264]
your new red white blue medic
[266]
a card and a Medicare and you handbook
[268]
they send you a new card because it's
[270]
gonna show part a on one date and Part B
[273]
effective the next the new date so if
[277]
that's all you do you'll have Original
[279]
Medicare Part A for your Hospital Part B
[282]
for medical which is full coverage under
[284]
Medicare unless you'd make some other
[286]
choice now you could decide to add drug
[289]
coverage under Part D or maybe by
[292]
Medicare supplement policy to help pick
[294]
up the cost that Original Medicare
[296]
doesn't cover or you could also choose
[299]
to join a Medicare Advantage plan
[301]
Part C if one's available where you live
[303]
and you'd get all your Medicare coverage
[305]
and sometimes extra benefits like vision
[308]
hearing dental and more bundle all
[311]
together in one plan so let's take a
[314]
look at that online application process
[317]
because it really is pretty easy so here

I'm just pulling up the web browser and

I'm just gonna go to Social Security
[328]
websites less SSA govt and you see here
[335]
on the Social Security website down here

and the home page it says Medicare
[340]
enrollment I just need to click right
[341]
there and this is their description on
[346]
Medicare benefits through Social
[348]
Security office they handle processing
[350]
all your forms slowly scroll down and as
[356]
we keep going there's two buttons here
[359]
we're gonna go past the first one
[362]
because the first one says how to apply
[364]
online just for Medicare only use this
[368]
when you're new to Medicare and you're
[370]
signing up for both a and B new at the
[373]
same time but if you keep going down
[376]
beyond that it says oh are you already
[379]
enrolled in Medicare
[381]
if you've already enrolled Medicare and
[383]
you want to start Part B fill out these
[386]
forms and here is the link apply online
[390]
for Medicare Part B during a special
[393]
moment so if you click that link and
[395]
again we'll have it separately for you

in the description box of the video

it will take me here and again this is

on the Social Security website let's see

if we can make this just a little bit

bigger there we go

so it says Medicare Part B enrollment
[416]
during the kovat 19 pandemic and again
[420]
you can read through the descriptions it

says to complete this online enrollment
[424]
you will need your Medicare number your
[427]
current address and phone a valid email
[429]
address and some form of documentation

one of those six ways we showed you the
[434]
presentation to verify group health
[436]
coverage to you or your spouse's current
[439]
employment now this is important right
[443]
here you will need to digitally sign the
[448]
form to complete the application to
[451]
complete the digital signature you will
[453]
need to provide an email address you
[455]
will receive an email from echo sign at
[458]
echo sign.com asking you to confirm your
[462]
digital signature if you do not receive
[465]
the confirmation email within a few
[468]
minutes of submitting your email address
[470]
please check your junk folder in case in
[474]
bold your signature is not complete and
[478]
your application will not be processed
[481]
until you complete the instructions in
[484]
your email so please folks take a notice
[487]
of that if you're doing this online
[489]
there's also a handy little tip that
[491]
this process online works best with
[494]
either Microsoft edge or Google Chrome

browser

so as you continue to go through

there'll be some things you need to
[502]
agree to if you're ready to go check the
[505]
box and start application

and what you'll see is a DocuSign box

and if you can click the start button

and follow along it will take you

through so this is your CMS for TB so

you should have a Medicare card with
[528]
Part A on it so it's going to want your
[530]
Medicare claim number I'm just gonna
[532]
type in a made-up number here hit next

do you want to sign up for Medicare Part

B yes I do next my name ok last name

first let's have fun with this and just

keep going mailing address

put in your phone number and here in my

remarks again I want my coverage

I want my Part B coverage to start let's

say today is July 10th fan let's say

your employer coverage runs through the
[587]
end of this month you want it to a start
[590]
August 1 so I want to start Oh 8 to 0 to
[594]
0 and again you can go as many as 60 day
[597]
three days out just keep hitting next

now it's asking for that employers
[604]
attestation this is that l 564 so I work
[607]
for acne and my employers address and
[618]
the applicants name if you aren't the
[620]
employee you could have had coverage
[623]
under your spouse you would be the
[625]
applicant name if you're the employee
[627]
and the Medicare beneficiary then
[630]
applicants name and employees name would
[632]
be the same so I am Frank and I put in
[642]
my social security number no dashes and
[646]
I had my coverage under my spouse
[648]
cynthia further and i'll put in her
[653]
social security number again no dashes
[655]
just keep hitting next were you covered

under the group health plan yes I was if
[666]
yes date coverage began now if you've
[668]
been there for twenty thirty forty years
[669]
something like that they don't want the
[671]
exact date twenty thirty years ago they
[674]
will accept the first of this year and
[678]
careful on the formatting they just want
[681]
the month two-digit month slash and then
[685]
that four-digit year has the coverage
[688]
ended yes or no well if you're still
[691]
covered under it put now if yes click
[694]
the date I'm just going to leave mine
[695]
blank

and when did you or your spouse work for
[700]
the company well we started there back
[702]
in August
[703]
/ 1985 and we are going until August
[711]
2020 so then it's got an employer
[715]
verification so again if you have your
[721]
documents on your computer and it could
[723]
be as simple as taking a photograph of

that pay stub that ID card that shows an

effective date then you'll need to have
[731]
them on your computer just come here

click to attach and then just know where
[737]
it's located on your computer to upload
[739]
it's that easy folks once you're all

done hit your sign and again you will
[750]
need to have a mail address get my apply

now in this case here it didn't let me

keep going because I didn't attach

anything but again it will send you an
[767]
email let you know that you need to
[768]
respond to that to complete the digital
[771]
signature process but after that you're
