[0]
in this video we are going to cover the
[1]
test battery by luma Oki to assess
[4]
movement control dysfunction in patients
[6]
with nonspecific low back pain hi
[14]
welcome back to physio judo's almost
[17]
everyone experiences low back pain at
[19]
least once in a lifetime only 10% of
[22]
cases of patients suffering from low
[24]
back pain can be attributed to specific
[26]
pathology we are talking about fractures
[29]
tumors
[30]
spinal stenosis neuropathology and such
[33]
the other 90% of cases are nonspecific
[36]
meaning there is no path or anatomic
[38]
cause for the pain nonspecific low back
[42]
pain can further be subdivided in non
[44]
mechanical and mechanical pain in the
[48]
non-mechanical group can be attributed
[49]
to central maladaptive processes
[52]
influenced by the presence of yellow
[54]
flags and other psychosocial factors
[56]
thumbs up if you want a video explaining
[59]
different forms of pain this
[62]
non-mechanical low back pain makes up
[64]
around 1/3 often on specific cases the
[67]
other 70% are mechanical and either
[70]
movement or posture dependent oftentimes
[73]
nothing is structurally wrong the
[75]
patient simply has lost control over the
[77]
conscious movements in the low back
[79]
leading to pain blue myopia doll has
[82]
developed a sixth test screening tool to
[84]
assess movement control dysfunction in
[86]
patients with nonspecific low back pain
[89]
they found substantial intra and
[92]
integrated reliability with a kappa
[94]
value of greater than 0.6 secondly there
[97]
was no significant difference between
[99]
novice and experienced clinicians rating
[102]
the patient's movement furthermore when
[104]
low back pain patients perform these
[106]
tests there was a clear significant
[108]
difference in movement control to
[110]
healthy subjects the first test of the
[113]
battery is the so called way dose Bo it
[115]
assesses control over the lumbar spine
[118]
during a hip hinge the goal is to bend
[120]
forward from the hips whilst maintaining
[122]
a neutral lumbar spine
[124]
secondly the patients are instructed to
[127]
perform a posterior pelvic tilt which
[130]
induces flexion at the lumber spine
[135]
third is single leg stance the patient
[139]
is instructed to stand with the feet
[141]
apart at one third of the inserter
[143]
cancer distance then the patient
[146]
proceeds to stand on one leg the amount
[149]
of lateral shift is measured the norm is
[152]
usually 8 centimeters and the
[153]
discrepancy of more than 2 centimeters
[155]
between sides is considered abnormal the
[158]
next test is sitting the extension for
[160]
the test the patient is an upright
[162]
sitting position on the bench with the
[164]
legs hanging freely the lumbar lordosis
[166]
is corrected and then the patient is
[169]
instructed to extend the knee without
[171]
movements ie flexion of the low back
[179]
next the patient's movement control is
[181]
assessed during forward and backward
[183]
rocking and 4-point kneeling position
[185]
for forward rocking the patient is
[188]
instructed to move the pelvis forward
[190]
without extending the low back for
[198]
backward rocking the patient is asked to
[200]
transfer the pelvis backwards whilst
[203]
keeping the back neutral
[213]
the last test is the pro knee bend the
[217]
patient lies in prone position and is
[219]
asked to bring the heel to the bottle
[230]
typically the weight of SPO sitting knee
[233]
extension and backward rocking are
[235]
difficult to perform rotations with a
[238]
flexion pattern according to ourselves
[240]
on the other hand a patient with an
[242]
extension pattern will have difficulties
[245]
with executing the prone knee bent and
[247]
forward rocking without involving the
[249]
lumbar spine
[250]
at last the single leg stance could help
[253]
you identify someone with a lateral
[255]
shift pattern what can you do with the
[259]
patient once you've figured out their
[261]
movement control dysfunction be sure to
[264]
check out the video right here to learn
[265]
about some interventions as always
[268]
thanks for watching and if you haven't
[269]
subscribed yet now is the chance
[271]
this was Andrea Fujii Oh toodles I'll
[273]
see you in the next video bye
