Safed Motiabind (Cataract) and Kala Motiabind (Glaucoma)

are two different diseases.

Safed Motiabind (Cataract) means

the lens has turned white,

whereas Kala Motiabind (Glaucoma) means

increased pressure inside the eye.

Kala Motiabind is known as Glaucoma

and Safed Motiabind is known as Cataract.

Let us first understand what they are,

whether you need to be worried about it,

what latest treatments exist,

what latest lens options you can have

that is used during the surgery,

Cataracts are very common

as you get older.

Just as hair turns grey with age,

when you grow older than 60 or 70 years,

the natural lens inside your eye

that is as clean and clear as a mirror

turns white when are older

that is known as Safed Motiabind

or Cataract.

It is not necessarily always age-related,

but most people get it as they grow older.

sometimes it can occur in young children,

in newborn infants,

due to certain medication,

due to diabetes or

due to an eye injury.

These may cause the lens to turn white.

These situations are rare;

for most people, it happens as they age.

Just as grey hair can come with age,

the lens can also turn white with age.

How can you identify a cataract?

If your lens gradually turns white in color

(first a faint white, then opaque),

The light entering the eye

is partially blocked due to this white layer.

and light is not able to reach the Retina properly

The symptoms that you first notice are:

you vision becomes blurred or hazy

changes in the prescription of your glasses,

particularly when driving at night

you see glare around headlights

(bright lights spread out too much)

or sometimes you experience double vision.

For example, if you are looking at the moon, you may see two moons.

If you notice any such symptoms and

if you are above 60 years of age,

there's a high change that

you may have Cataract.

For cataracts, it's important to note that

the only treatment option available for Cataracts

During a Cataract Operation,

the natural lens that has become white & clouded is removed

nowadays, the cataract surgery and artificial lens used

both have seen many advancements and changes.

The artificial lenses used today

are designed in such a way that after the operation,

there is no need to wear eyeglasses.

These days, we combine Cataract surgery with a Refractive Surgery

in such a way, that

the operation can remove both cataracts and need for power glasses.

Latest Cataract surgeries done today

don't involve stitches or pain.

The entire surgery is done using numbing eye drops.

There are no stitches, no pain, no injections or no issues;

the entire surgery is performed using only numbing eye drops.

Hence, the patient doesn't feel any pain during surgery.

There are three categories of cataract operations.

Let me first explain how the cataract (cloudy lens) is removed?

The cloudy lens (cataract) can be removed by 3 types of operations
[368]
and there are 4 categories of artificial lenses inserted in the eye after removing the cloudy lens.

I'll explain this in detail.

First, the basic operation done today in Delhi,
[382]
(Earlier we had operations like SICS, ECCE
[386]
but nowadays they're no longer popular
[388]
in big cities like Delhi
[390]
because they involve making a large incision.)

Modern methods use microscopes and small incisions
[396]
The first type of operation is MICS
[400]
(Micro Incision Cataract Surgery).
[404]
MICS is a minimum standard now
[408]
We don't prefer surgeries older surgeries
[410]
MICS involves three machines.
[414]
One is the Centurion machine, the latest,
[417]
along with an Optical biometer
[419]
which is used to calculate lens power
[422]
and a high-end display microscope.
[424]
In Delhi this is a basic-level surgery,
[429]
but the majority don't opt for this.

It has lost popularity since the advent of Laser cataract surgery.

Next type of surgery is Zepto surgery.

As you can also see here,

hard cataracts or mature cataracts
[449]
that look either white or black
[452]
are removed with Zepto Laser Technology.

This surgery is only for a few people.

Doctors usually suggest

to not let a cataract mature.

Earlier, doctors would suggest waiting for cataracts to be mature before getting operated

Why would they say that?

Because the older concept of cataract was

that the lens that has turned white
[484]
must first completely mature.

Then doctors would make a large incision
[491]
to extract the entire lens as is
[494]
and insert a new one in its place.

Doctors today don't recommend this

due to the large incision that it needed

which must then be closed

with six to seven stitches.

Modern operations are stitch-free.

Methods used are microscope and laser

However, there is one condition, to be eligible for modern surgeries

that cataract should not be hard/matured.

If the cataract matures, it hardens.

The harder it is, the more laser is used to break it

which can eventually harm eye tissue.

The latest guidelines and understanding

are that the cataract must not mature;

it must be extracted when fresh,

as soon as you start having difficulty

with your daily activities.

For example, working on a computer,

or a housewife unable to do her chores.

When it starts to affect your daily work,

get the surgery done.

However, these are also losing popularity;

The most popular cataract surgery today is

Femto Laser with Artificial Intelligence.

This latest technology won't require

eye glass after surgery.

However, Indian lenses are also now high quality.

The second type of lens is called

But for near sight work

This means that at if you go out at night

Because this can be quite uncomfortable,

Multifocal lenses are mostly not advisable.

Exceptions are housewives or retired people

Multifocal lenses are used for less than 1% of all patients.

Multifocal lenses are also expensive,
