dentistry. And this video is on a rather聽 frustrating topic: Why we are dropping a聽聽

lot of PPO dental insurances. We're not dropping聽 them per se, we're going out-of-network. What聽聽

does that even mean? With all the political聽 talk right now about health care, Obamacare,聽聽
[26]
what not, be aware that healthcare and dental聽 health care benefits are changing. I've been聽聽
[34]
practicing for over 15 years, and the amounts聽 that were paid for a procedure are the same聽聽
[41]
or less than they were 15 years ago, which is聽 pretty staggering. Now as a biologic dentist,聽聽

and I'd like to admit, a very high-tech dentist, I聽 use nothing but the best materials. I have a very,聽聽
[56]
very expensive 3D CAD CAM machine, we have lasers,聽 we have the lowest radiation, brand-new digital聽聽
[63]
radiography units, we have aromatherapy, we have聽 a lot of things that we offer in this office that聽聽

we're not paid for. The burden as a business owner聽 is that I'm actually losing money treating a lot聽聽

of patients in-network. Believe it or not, some聽 exams for me to spend an hour with a patient,聽聽

I'm reimbursed $17. Now some patients are like,聽 'Yes she gets $17 but then they probably the聽聽
[91]
insurance kicks in another $300!' No, I literally聽 get $17. So not only is my time free, I'm actually聽聽
[100]
losing money on the assistant sitting next to聽 me who's charting. That's rather frightening.

Also with the dental materials. We're paid聽 so little that, because I use such high-end聽聽
[111]
materials, I actually break even with what the聽 insurance reimburses me just on my material聽聽
[117]
costs which means, again, I'm working for聽 free. I don't like to rush my procedures. I聽聽

love to spend time with my patients. I love to聽 do a really thorough new patient intake exam.聽聽

And instead of shortening it to 5 minutes, which聽 is what insurance would require, and having six聽聽
[136]
patients in the waiting room all waiting an hour聽 each - I can't afford to do it for $17 an hour.

Now what does this mean? I'm not going to聽 change the way I practice. What's going to聽聽
[148]
make this practice right for you is someone who聽 really values having the best of everything,聽聽
[153]
at a reasonable, affordable price! We haven't聽 raised our fees at all. We're actually very low聽聽
[158]
for our area. We're in the 70th percentile for聽 our area. So I want to make dentistry affordable,聽聽

and what it means being out-of-network means聽 that we will still bill your insurance,聽聽
[168]
we will get insurance breakdown, we're聽 well versed and all of your insurance聽聽
[172]
needs and requirements, and we'll聽 submit the claims for you. However,聽聽
[176]
they're going to reimburse us at聽 our fee versus the fee they've set.

For example: A crown typically is about聽 $1100, $1200. Insurance will only pay us聽聽
[190]
$598 in-network. And then, if you get half of聽 that paid for by insurance, your co-payment聽聽
[197]
would be about $300. So now you would have聽 a slightly larger co-payment, the insurance聽聽
[204]
would still pay for half of the crown, it would聽 just be it our set fee versus the fee of $600.

So now you're saying, 'Oh gosh, well now聽 I can't even go to the dentist... I can't聽聽
[217]
go to the dentist because she's going to聽 charge me so much money.' That is the most聽聽
[220]
common misconception people have when they聽 hear "out-of-network". Now other than HMO,聽聽
[225]
which is where you're required to see a聽 dentist has to be on their network paperwork,聽聽
[229]
who is in-network with HMO. With PPO insurances聽 you can go wherever you want, and you can pay聽聽
[236]
just a little more out-of-pocket. Sometimes聽 it's as little as 10% more. It's not double,聽聽
[241]
it's not triple, it's not quadruple.聽 It literally is as little as 10% more.

The other thing to consider is that I'm a really聽 conservative dentist, who... I really pride myself聽聽
[253]
on being a patient advocate. So part of that is,聽 in order to compensate for low fees, I'm not going聽聽
[259]
to upsell you on anything. I'm not going to tell聽 you about stuff that you need or don't need, I'm聽聽
[264]
just going to tell you about what's important for聽 your health, and then we'll have a conversation聽聽
[268]
about what might be needed in the future.聽 So instead of having a big dental bill dump,聽聽

which is what most people anticipate when they go聽 to the dentist, what they don't realize is that a聽聽
[279]
lot of these in-network low network PPO insurance聽 practices are driven with a practice model where聽聽
[286]
they have to upsell you on stuff. So there's聽 so many hidden charges on things. It's kind聽聽
[291]
of like you know you buy your airline ticket聽 now they're going to charge you for a snack,聽聽
[296]
and for your bag, and based on your bag weight.聽 And they're trying to essentially just recoup聽聽
[300]
some of their costs to try and keep it competitive聽 fare. In dentistry it's the same thing. There's a聽聽
[306]
lot of unnecessary charges at every single聽 procedure that dentists tack on, just so聽聽
[312]
they can try and build up the fee to something聽 close to what they need to stay in business.

So essentially in order to聽 give people the best care,聽聽
[323]
we are still in-network with several聽 insurances but we'll be out-of-network聽聽
[327]
with those we accepted previously聽 that just have too low of a fee.

The other complicated thing is that my referral聽 specialists, I work very closely with a specific聽聽
[337]
team of highly trained oral surgeons,聽 root canal specialist if you need one,聽聽
[342]
periodontal specialists that are very aligned聽 with my way of thinking. And they're also very聽聽
[348]
conservative and they have an excellent聽 chairside manner. None of them anymore聽聽
[353]
are taking some of these insurances because for聽 them the reimbursement is also so low. So then聽聽
[359]
patients are just left going to the telephone book聽 in the old-fashioned way or essentially Googling聽聽
[365]
who's going to take their insurance, calling聽 their insurance network, and then randomly聽聽
[369]
throwing a dart at the dartboard to figure out聽 who's going to take their insurance from them.

So essentially whatever someone needs, I'm聽 happy to talk to them about. We have a really,聽聽
[381]
really great financial coordinator / treatment聽 planner that can explain all of your options聽聽
[385]
and give you a very clear estimate of what聽 your dental insurance is going to pay for聽聽
[390]
and what your out-of-pocket will be. It'll also聽 show you what the hidden downgrade fees are,聽聽
[394]
meaning that instead of a white filling聽 insuranceswill still only pay sometimes聽聽
[398]
for a mercury filling, or instead of a white聽 crown they'll only pay for a base metal or聽聽
[402]
low base metal crown... they're going to go聽 over all those nuances about your insurance,聽聽
[407]
so you really know what your out-of-pocket聽 would be. And again my only priority is the聽聽
[412]
patient and patient care. It's not聽 the business of dental insurance.
